How to map Syst(spec)?

1981003000101 |Arbitrary concentration of severe acute respiratory syndrome coronavirus 2 RNA (observable entity)|

Inheres in → Systemic part of is incorrect, but what should it be if anything?

We have had a discussion oh how to represent the context on “system” sometime earlier on och I don’t think we made a general consensus to what would be the accepted definition (context) to be represented by SNOMED.

In the discussion, it was mentioned that there is the in-vivo system and the in-vitro system, though for NPU the in-vivo system dominates. Is that correct? Can we create two SNOMED codes to represent these two states? Would that correctly represent the sample/test?

Information based on Young Bae´s email on the 15 March 2025.

NPU57165 P—Triacylglycerol lipase; cat.c.(37 °C;proc.) = ? U/L

FSN: Catalytic activity of triacylglycerol lipase in plasma in U/L with reference to 37 °C (observable entity)

  • catalytic activity concentration
  • how to phrase “37 °C”?

NPU56087, NPU29745, NPU21371 are genetic and should be left out?

NPU53029 Syst(spec.)—Neisseria gonorrhoeae(DNA); arb.c.(proc.) = ?

FSN: Arbitrary concentration of Neisseria gonorrhoeae (observable entity)

Should FSN include “in specified system” (or similar)

NPU29478 F—Haemoglobin; mass fr. = ? × 10-6
FSN: Mass fraction of haemoglobin in faeces in 10^-6 (observable entity)

Unit phrasing is a bit off?

NPU28874 B—Phosphatidylethanol(16:0/18:1); subst.c. = ? µmol/L
FSN: Substance concentration of phosphatidylethanol in blood in umol/L (observable entity)

  • 16:0/18:1 is missing

NPU28402 P—Connective tissue disease related antibody; arb.subst.c.(proc.) = ? (p.d.u.)
FSN: Arbitrary substance concentration of ribosomal ribonucleoprotein antibody in plasma in arbitrary unit (observable entity)

  • Confirm that Connective tissue disease related antibody = ribosomal ribonucleoprotein antibody

NPU26880 Ercs(B)—Haemoglobin; entitic mass = ? pg
FSN: Entitic mass of haemoglobin in erythrocyte in pg with reference to Blood (observable entity)

  • | inherent location | = | blood | missing
  • Better phrasing?

NPU22069 P(fPt)—Glucose; subst.c. = ? mmol/L
FSN: Substance concentration of glucose in plasma in mmol/L with reference to fasting Patient (observable entity)

  • | precondition | = 726055006 |After calorie fasting (qualifier value)| missing
  • Phrasing “with reference to”?

NPU21571 P—Pro-brain natriuretic peptide(1-76); mass c. = ? ng/L
FSN: Mass concentration of N-terminal pro-B-type natriuretic peptide in plasma in ng/L (observable entity)

  • Add proBNP(1-76) as synonym of NT-proBNP?

Units are sometimes abbreviated, sometimes spelled out in FSN.
e.g. 1741003000105 |Mass concentration of tacrolimus in blood in ug/L (observable entity)| vs. 1831003000107 |Substance fraction of glycated haemoglobin-A1c in globin chain in micromole/millimole with reference to Blood (observable entity)|

PT of 427323004 |Micromole/millimole (qualifier value)| is the reason!

NPU14566 P—Transglutaminase antibody(IgA); arb.subst.c.(proc.) = ? (p.d.u.)

  • confirm that Transglutaminase antibody(IgA) = tissue transglutaminase IgA antibody

NPU10267 P—Calcifediol+25-Hydroxyergocalciferol; subst.c. = ? nmol/L
FSN: Substance concentration of 25-hydroxyergocalciferol and calcifediol in plasma in nmol/L (observable entity)

  • check alignment with LOINC

NPU10238 P—Apolipoprotein B/Apolipoprotein A1; subst.ratio = ?
FSN: Substance ratio of apolipoprotein A-I and apolipoprotein B in plasma (observable entity)

  • ratio using two components is not correct

NPU04169 P—Calcium(II); subst.c.(adj.;proc.) = ? mmol/L
FSN: Substance concentration of calcium electrolyte in plasma in mmol/L with reference to adjusted (observable entity)

  • phrasing of procedure “adjusted”?

2026-01-09:
Agree to add phrase “in specified system” to FSN when NPU system = Syst(spec)

  • Replace Catalytic activity with this: 118524006 |Catalytic concentration (property) (qualifier value)| in the map
  • “estimation at 37 °C” is a better wording, we need to collect examples where the same phrase is applicable
